Cleft mitral valve
Cleft mitral valve most commonly occurs as a component of an atrio-ventricular septal defect. Occasionally, it occurs alone as a single abnormality as seen here. In short axis view the mitral valve shows a division of its anterior leaflet (arrow) which results in mitral regurgitation. In conventional left ventricular long axis view, the anterior mitral leaflet will appear to dome during diastole, although there is no stenosis of the valve.

In short axis view the mitral valve shows a division of its anterior leaflet with resulting mitral regurgitation through the cleft in the anterior leaflet.
In apical long axis view of the left ventricle the mitral valve shows a division of its anterior leaflet with resulting mitral regurgitation through the cleft in the anterior leaflet.
